Application Procedure

1. Complete the online "application form" below by May 15.
   Also download and complete health form and photo release form.
   
2. Attach your $25 registration fee made payable to Blue Mountain Festival.
   
3. Send the forms and check to:

         PO Box 2373
         Boothwyn, PA 19061

*Late applications will be reviewed at the discretion of the executive director.
